Can Carvedilol be used in someone with COPD?

This is a case study.

The patient is a 62 y/o man complaining of decreased exercise tolerance. He suffered an MI 2 years ago for which he received PCI and a bare metal stent.. He has DM type II, COPD, and hyperlipidemia. Pertinent meds are verapamil ER 240mg daily, ISDN 30mg daily. He also takes NPH insulin for DM and albuterol and ipratropium for COPD. Also he is on aspirin 81mg daily.

His current ejection fraction is 30% and BP is 118/86. HR is 74 bpm, Scr is 1.4. BUN is 24 and Na and K are in normal limits.

I think the verapamil should be discontinued and replaced with carvedilol since it has been studied more in heart failure, but I’m worried about it affecting his COPD status. Can you tell me what suggestions you have? Please include references (or links) if you are using web resources.

What do you think of Obesity in America?

Do you think Obesity is measured correctly in America? I personally disagree with the BMI calculation and think it warps what defines obesity. If for example, you’re a 230 Ib. 6′1 male who can hardly pinch fat because of muscle, you’re still defines according to BMI as overweight! Who thought up this crap? And as I asked before, what do you think about the subject in general?

My symptoms:Fatty liver disease, LQT and ?ischemia, wheat/diary/egg/yeast/tomato allergies, fatigue, obesity, navil hernia, sleep apnea, high BP, vaginal itch, sleep problems, eczema on head, ears ?infection all the time, lump in left arm pit and left vagina, aches esp knees and back, breathing difficulties, thinning hair, bloated, wake up worse in morning, falling asleep at 4pm cant drive then, dizzy, poor short term memory, depression after anxiety, pins and needles in both hands and right leg/foot when asleep chest pains and shortage of breath.What do I have?

Do you think it’s the governments responsibility to make changes to help fight obesity in America?

I’m not sure how I feel about the issue and although obesity is a major problem, I don’t understand why resturants should be forced to change their menus and ways of cooking because of it. People have a choice to eat what they want..If they choose to eat unhealthy than they pay the consequences for it.

Does morbid obesity affect brain function?

Shouldn’t we trust the views of someone like Rush Limbaugh who has obvious hypernutrition rather than an obvious fit and healthy freak like Obama? I mean doesn’t one’s physical health impart some effect on one’s values and cognitive ability? People who weigh more and achieve morbid obesity are able to provide more fuel for their brains, aren’t they?
